Job Summary AMBULATORY CARE MEDICATION MANAGEMENT SPECIALIST- Wellness Programs (hybrid role) Novant Health is seeking a Medication Management Specialist, to join our ambulatory pharmacy team, to deliver optimal medication management across all dimensions of care, every time. This role will have a dual focus: (1) serving as a dedicated resource for Anti-Obesity Medication (AOM) management and (2) providing clinical and operational support for our Healthy Lifestyles Program. Preferred Experience/Credentials 5+ years of outpatient pharmacy or managed care Customer service oriented; excellent interpersonal and written communication skills Highly motivated individual who is self-directed, adaptable and able to execute tasks in a quickly changing environment Strong knowledge of prior authorization, step therapy, and benefit coverage processes Familiarity with anti-obesity medications and payer coverage requirements Ability to multi-task, prioritize essential tasks and meet deadlines Requires knowledge of prescription assistance programs and available resources to reduce cost of medications Remote opportunity within our approved hiring footprint. Will need to come on-site as needed. Essential functions include: Serve as dedicated anti-obesity medication (AOM) resource Perform comprehensive medical chart and medication reviews for medically complex patients, particularly in the weight management space Manage intake and end-to-end review of AOM prescriptions, ensuring consistent application of criteria across workflows Implement and maintain SOPs to drive standardization and improve efficiency Communicate with team members and dependents on the Novant Health medical plan to explain coverage determinations and next steps Confirm coverage under benefit design; assess for manufacturer/patient assistance resources; document and communicate determinations clearly Healthy Lifestyles Program Support Act as a utility resource for the Healthy Lifestyles Program, assisting with clinical pharmacy components (e.g., chronic disease management, medication adherence, member education, Conduct outreach, including contacting patients and scheduling appointments as needed. General Medication Management Support Partnership with Novant Health providers and clinical staff to provide remarkable patient care. Obtain medication histories, including fill history, for medically complex patients. Addresses insurance, reimbursement, and payment issues as appropriate by communicating with insurance providers, patients, and pharmacists. Experience: Minimum of five years experience in pharmacy, required. Licensure/Certification: PTCB Certification and NCBOP License, required. Additional Skills (required): Considerable knowledge of medication names (both trade and generic) and their doses, routes of administration and adverse effects. Demonstrates patient interview skills and strong written and verbal communication skills with other healthcare professionals. Ability to follow verbal and written instructions. Basic technology skills. Other duties asoutlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed.
