Education

Master’s Degree – Social Work

Certification/Licensure

Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hireCertified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hire

Experience

1 year of Case Management or other related experience

Duties/Responsibilities

Responsible for the provision of medical social work services to patients receiving care in the hospital setting.Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs for each referred patient.Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community.Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care.

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$53,248.00-$88,753.60. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
