Medical Records Technician (Coder) Auditor
Summary The Medical Records Technician - Auditor is a technical position responsible for maintaining, reviewing, and analyzing medical records to ensure accuracy, completeness, and compliance with regulatory standards. This is part of Electronic Health Records Management (EHRM). The 2-page Resume requirement does not apply to this occupational series. For more information, refer to Required Documents below. Responsibilities Total Rewards of a Allied Health Professional The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH. The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division. Auditors in this role perform prospective and retrospective coding audits, identify coding and documentation issues, provide feedback and training to clinical staff, and use computerized applications to generate reports and validate data at the Cincinnati VAMC. Other duties include: Reviewing records for deficiencies, such as missing signatures, incomplete reports, or other errors, in both paper and electronic health record (EHR) systems. Auditing assigned medical codes (ICD-10, CPT, HCPCS) for accuracy and completeness, ensuring they are supported by the clinical documentation. Analyzing audit results and other medical record data to identify patterns and trends, which are then used to develop recommendations for process improvements. Abstracting and compiling clinical and administrative data from medical records to support quality assurance, utilization review, and other health information management functions. Providing expertise, advice, and education to medical staff and coders on documentation requirements, coding rules, and compliance. Ensuring that medical recordkeeping practices and coding align with regulatory requirements, such as HIPAA, and standards from accrediting bodies like The Joint Commission. Preparing reports on audit findings, data trends, and quality metrics for distribution to medical staff, management, and committees. Researching and resolving complex documentation and coding issues that arise, which helps to improve data accuracy and quality. Other duties as assigned. Work schedule: Monday through Friday, Tour of duty to be discussed at the time of interview. Compressed/Flexible Schedule: To be discussed once all training has been complete and per the needs of the service. Remote: This is a 100% remote position. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Experience: One year of creditable experience that indicates knowledge of medical terminology, anatomy, physiology, pathophysiology, medical coding, and the structure and format of a health records; OR Education: An associate's degree from an accredited college or university recognized by the U.S. Department of Education with a major field of study in health information technology/health information management, or a related degree with a minimum of 12 semester hours in health information technology/health information management (e.g., courses in medical terminology, anatomy and physiology, medical coding, and introduction to health records); OR Completion of an AHIMA approved coding program, or other intense coding training program of approximately one year or more that included courses in anatomy and physiology, medical terminology, basic ICD diagnostic/procedural, and basic CPT coding. The training program must have led to eligibility for coding certification/certification examination, and the sponsoring academic institution must have been accredited by a national U.S. Department of Education accreditor, or comparable international accrediting authority at the time the program was completed; OR Experience/Education Combination: Equivalent combinations of creditable experience and education are qualifying for meeting the basic requirements. The following educational/training substitutions are appropriate for combining education and creditable experience: Six months of creditable experience that indicates knowledge of medical terminology, general understanding of medical coding and the health record, and one year above high school, with a minimum of 6 semester hours of health information technology courses; OR Successful completion of a course for medical technicians, hospital corpsmen, medical service specialists, or hospital training obtained in a training program given by the Armed Forces or the U.S. Maritime Service, under close medical and professional supervision, may be substituted on a month-for-month basis for up to six months of experience provided the training program included courses in anatomy, physiology, and health record techniques and procedures. Also, requires six additional months of creditable experience that is paid or non-paid employment equivalent to a MRT (Coder). Certification: You MUST have a Mastery Level Certification through AHIMA or AAPC. Current mastery level certifications include: Certified Coding Specialist (CCS), Certified Coding Specialist - Physician-based (CCS-P), Registered Health Information Technician (RHIT), Registered Health Information Administrator (RHIA), Certified Professional Coder (CPC), Certified Outpatient Coder (COC), Certified Inpatient Coder (CIC). Experience: You must have one year of creditable experience equivalent to the GS-8 grade level of a MRT (Coder). Candidates must also demonstrate all of the knowledge, skills, and abilities below: Advanced knowledge of current coding classification systems such as ICD, CPT, and HCPCS for the subspecialty being assigned (outpatient, inpatient, outpatient and inpatient combined). Ability to research and solve complex questions related to coding conventions and guidelines in an accurate and timely manner. Ability to review coded data and supporting documentation to identify adherence to applicable standards, coding conventions and guidelines, and documentation requirements. Ability to format and present audit results, identify trends, and provide guidance to improve accuracy. Skill in interpersonal relations and conflict resolution to deal with individuals at all organizational levels. Physical Requirements: Office setting; moderate lifting and carrying, reaching above shoulder, use of fingers, and periods of standing/sitting.
