Essential Duties and Job Responsibilities
Organize and maintain paper and/or electronic health records, ensuring they are complete, accurate, and easily accessible. Follow facility and regulatory guidelines for medical records storage, retrieval, and disposal. Enter and update resident information, including demographic data, diagnoses, procedures, and medications. Retrieve medical records for medical staff or other authorized individuals. Review records for completeness, accuracy, and timeliness, identifying and addressing discrepancies. May assist with data collection, analysis, and reporting for quality improvement or other facility purposes. Adhere to HIPAA regulations and maintain the confidentiality of patient information. Ensure the accuracy, completeness, and timely completion of medical records, including completion of discharge sheets and 360 care documentation. Maintain accurate records of all actions taken, including record releases, audits, and system updates. Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator. Must be knowledgeable on federal and state laws regarding medical records. Review and audit admissions, discharges, change of condition, and other pertinent records on nursing units. Ensure that incomplete records/charts are returned to nursing service for correction. Ensure that registries are properly maintained for admission and discharge of residents. Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility to assure that medical records can be properly maintained and problem areas can be identified and corrected.Requirements and Qualifications
High school diploma or equivalent. Basic understanding of medical terminology and anatomy/physiology. Proficiency in Microsoft Office Suite and/or electronic health record (EHR) systems. Experience in medical records or healthcare administration is preferred. Excellent communication and interpersonal skills. Strong organizational and multitasking abilities. The ability to work in a fast-paced, crisis-prone environment. Advocacy skills with a focus on patient-centered care.At Avamere, we believe in taking care of our employees.
