cannot provide the advanced care expertise and new treatment options their patients require they turn to usJob SummaryProvide case management to clients with multiple medical and psychosocial needs Coordinate psychosocial support services with community based social and substance abuse agencies and case managers To increase client entry into and retention within medical care while improving each clientrsquos overall quality of life through the development and monitoring of practical and realistic plans of careJob ResponsibilitiesMaintain a caseload HIV clients consistent with requirements of the Ryan White ContractTriage and assignment of incoming clients maintain a waitlist as appropriateProvide back up coverage for other MCMs who may be out on vacation or on extended leaves of absence when necessary This will be done in conjunction with the Program Manager and the Physician LeadDetermine client eligibility for Ryan White funded services including financial assistance and budgetingConduct a comprehensive intake with particular emphasis on medical related needs and other needs that may jeopardize a clientrsquos optimal health outcomes consistent with the requirements of the Ryan White ContractPartner with each client to create an individualized Plan of Care that addresses the needs of each client and attempts to alleviate real and potential barriers to medical care and medical complianceReview each clientrsquos medical status including CD counts viral loads medication changes and type and severity of symptomsConnect clients to appropriate medical supportive and entitlement services that have been identified in the plan of careAdvocate on behalf of clients for the removal of barriers to quality medical careMaintain complete orderly and updated client files in compliance with HIPAA regulations Ryan White Part A procedures and State of Connecticut Ryan White MCM Standards of CareDevelop a working knowledge of government entitlement programs AIDS assistance programs and community social service programsTimely entry of activity into the CAREWare automated systemComplete program reports as necessaryServe on agency committees as requested by supervisorLocal
