The Primary Care Nurse (PCN) is in charge of care for a specific group of elders, to include supervision of team of CNA. Nursing services delivered by the CNAs are supervised by and directly accountable to the Manager LTC Nursing, and DON through the licensed nurses in the lodges. The PCN coordinates referrals to other members of the clinical support team, families, and providers. The PCN serves as the point of contact for elder information.

Required Qualifications:
Coursework/Training: Graduate of an accredited School as a License Practical Nurse. Upon hire: Alaska Licensed Practical Nurse. Upon hire: National Provider BLS - American Heart Association.Preferred Qualifications:
6 months Experience in a health care facility preferred (acute care, long term care, rehabilitation center).
