Position Purpose:
The Medicaid Eligibility Specialist plays a vital role in supporting participants and families through the Medicaid application and re-determination process. This position ensures timely and accurate submission of documentation to county agencies and maintains detailed records of eligibility status.

Key Responsibilities:

Assist participants and caregivers in initiating and completing Medicaid applications.Conduct home visits to gather necessary documentation and submit cases within approved timeframes.Complete all Medicaid re-determinations at least 30 days prior to due dates.Communicate Medicaid rules and options (e.g., LTC Medicaid, Spousal Impoverishment, Trusts) to families and participants.Coordinate disenrollment from HMOs and update enrollment specialists and leadership.Maintain confidentiality of participant and family information.Collaborate with internal teams to ensure a warm, supportive environment and seamless service delivery.Stay current on Federal, State, and local Medicaid regulations and Trinity Health policies.Participate in required training and contribute to a culture of continuous improvement.

Qualifications:

High school diploma or equivalent required.Minimum one year of experience working with elderly or frail populations.Strong knowledge of Medicaid systems, benefits, and community resources.Excellent communication and interpersonal skills, with the ability to work independently.Proficiency in Microsoft Office and other relevant software.Strong organizational and problem-solving abilities.Ability to manage multiple tasks and adapt to changing priorities.Must be able to travel locally as needed and meet physical requirements of the role.
