MDS Nurse (LPN or RN) – Cincinnati, OH

We are seeking a detail-oriented and motivated MDS Nurse (RN or LPN) to join our skilled nursing team in Cincinnati. This role plays a key part in ensuring accurate and timely completion of resident assessments and supporting our commitment to quality care.

Responsibilities:

Coordinate and oversee the Resident Assessment Instrument (RAI) process.
 Accurately complete and submit MDS assessments within required timelines.
 Work collaboratively with the interdisciplinary team to ensure compliance with state and federal regulations.
 Monitor care plans and provide input for ongoing improvement in resident outcomes.
 Maintain knowledge of current reimbursement and regulatory guidelines.
 

Qualifications:

Licensed Practical Nurse (LPN) or Registered Nurse (RN) with active Ohio license.
 Previous MDS experience preferred, but we are willing to train the right candidate with strong clinical and organizational skills.
 Knowledge of state/federal guidelines and reimbursement practices a plus.
 Strong attention to detail, communication, and teamwork skills.
