Our Nursing team at Eddy Memorial Geriatric Center is seeking a full-time MDS Coordinator for our 80-bed Skilled Nursing facility in Troy, NY!

The RN MDS Coordinator will be responsible for, but not limited to:
Oversee all aspects of Medicare requirements and MFD coordinationResponsible for ensuring that the MDS is submitted timely and accurately reflects the services providedWork with others to coordinate communication among all clinical staffCoordinates Medicare PDPM, PPS and OBRA care plan schedules for the facilityWork collaboratively with facility MDS RN Specialist and SPHP MDS TeamResponsible for ensuring completion of required documentation for Medicare residents (admission through discharge)Monitor MDS data and other documentation for accuracyParticipate in determining Medicare Part A eligibility and other MDS considerationsAct as a MDS/PPS resource to the interdisciplinary teamPerforms electronic submission of the facility-wide MDS information timely to the state of New YorkActively participates in resident care planning providing CAAs reports to the nurse managers in database, coordinating with medical records for updating face sheetsEnters and maintains residents' ICD-10 code
Minimum Requirements:
Valid NYS RN licenseAssociate’s degree in nursingCurrent CPR and BLS or achieve with onsite trainingMDS Certification or must achieve within designated time period with the site's assistance in obtaining
Preferred Requirements:
Bachelor’s degree in nursingRAC-CT Certification preferred2+ years of experience as a RN in a skilled nursing facility or 5 + years in a direct care role (LPN, C.N.A)3+ years of experience as a MDS coordinator
Pay Range:$37.60 - $53.10
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
