Brooke Knoll Village is looking for a licensed LPN or RN to join our team as an MDS Coordinator.  Candidates should have received on-the-job training or completed an MDS-training program.  We are also willing to train quick learners. MDS Coordinators must be organized, detail-oriented, have great assessment skills and be able to work independently.

About the Role: Brooke Knoll Village is seeking an MDS Coordinator to-MDS scheduling and completion for OBRA/PPS/Managed Care utilizing RAI guidelines Assist with care plan scheduling and completion with the interdisciplinary teamAssign and update ICD-10 codes based off physician diagnosis on admission and with each MDSTransmit OBRA/PPS MDS Assessments to CMS per Federal GuidelinesCompletion and Certifications/Re-certifications when a resident is receiving Medicare Part A BenefitOversee and monitor MDS documentation and charting requirements needed to support services providedEducate staff on MDS processes as neededAssist with data collection for audits including but not limited to, State, Medicare, and Managed Care.Participate in facility and company meetings per policyBecome proficient with and apply RAI rules/regulationsBecome familiar with QMs and assist team in monitoring
About you: The ideal candidate would have the following skills and experience:Licensed LPN or RN in the state of IndianaOrganized, detail-oriented, have great assessment and communication skills and should be able to work independently and as teamAmbitious, inquisitive, and eager to learn
