Manager of Post-Acute Care Operations - Population Health

Primary Function/General Purpose of Position

The Manager, Post Acute Care Operations position is accountable for developing the professional practice model for post-acute care management to serve value based care goals for the organization. The scope of duties includes development of the primary RN team, governance, innovations, and workflows for episodic clinical care progression to maintain site appropriateness and improve patient outcomes in the care continuum.

This is a remote/work at home position. Hire must be open to working eastern time zone hours.

Essential Job Functions

In collaboration with internal and external stakeholders:

Analyzes assessment data.

Creates plans of care.

Monitors and evaluates treatment effectiveness.

Recommends appropriate clinical interventions and care settings.

Post-acute point of contact for the organization and market stakeholders including collaborative meetings, internal and external representation, and presentations.

Coordinates care across various post-acute sites and informs stakeholders of variances. 

Acts as a patient safety advocate by participating in ongoing quality improvement in the department.  Incorporates the professional practice model into care delivery.

Incorporates disparate sources of information and knowledge to facilitate integration of programming

Licensing/Certification

BLS Basic Life Support – American Heart Association (required)

Current RN License in the state in which they are working or covered by compact (required)

Care Management certification (preferred)

Education

Bachelor of Science in Nursing (BSN) (required)

Master's degree, Nursing or Healthcare Administration (preferred)

Work Experience

3 years of experience in transitions of care, care management, quality, or risk (preferred)

Previous Experience in care continuum settings (post-acute, home health, palliative) (preferred)

Training

EPIC Electronic Health Record (preferred)

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.​
