What You’ll Do:

Deliver direct, compassionate nursing care to hospice patients in their homes or facility settings

Collaborate with physicians, social workers, spiritual counselors, and other team members to create and follow individualized care plans

Educate patients and families on care processes and provide emotional support throughout the journey

What You’ll Need:

Active LVN license in good standing

Valid driver's license, active auto insurance, and a reliable personal vehicle for work-related travel

Responsibilities Provide direct patient care to hospice patients, including appropriate pain/symptom managementImplement nursing practice following a comprehensive assessment and plan of careProvide accurate and timely documentation of patient servicesAssess and provide patient and family/caregiver educationParticipate in coordination of hospice servicesProvide appropriate pain/symptom management and evaluate patient response to treatments/medicationsParticipate in professional growth and development opportunities and quality assessment activitiesEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation

Three years nursing experience. Community health/hospice or medical/surgical experience is

preferred

Knowledge of the hospice philosophy of careCommitment to clinical and documentation excellence

Must be a licensed driver with an automobile that is insured in accordance with state and/or

organization requirements and is in good working order.

Possess and maintain current CPR certification.
