Summary/Objective:Under the general supervision of a Registered Nurse
and reporting to the Executive Director, the Licensed Practical Nurse (LPN)
provides direct nursing care and clinical oversight for assigned
residents/tenants in accordance with Wisconsin DHS 83 and/or DHS 89
regulations. This role supports the health, safety, and well-being of residents
by monitoring medical conditions, administering medications and treatments,
documenting care, and collaborating with interdisciplinary staff to deliver
individualized, person-centered services. The LPN promotes resident dignity and
independence while ensuring adherence to facility policies, state regulations,
and nursing standards of practice

Perform as primary resource for coordination of clinical referral review in a responsive time frame with center leadership or liaisons in the field and support nursing manager to complete full admissions process including but not limited to:o Patient chart review
§ Diagnosis, Orders, Hospital EMR- H&P of resident stay, labs and X-rays
o Coordinate admission decision per clinical capabilities grid and Center operations
o Assist all Central Intake Coordinators with clinical decision making in certain circumstances
o Identify clinical opportunities per referral types
o Enter assigned tasks into EMR and communicate patient updates with building as needed
Respond to admission inquiries timely and follow North Shore Admission Acceptance/Denial Process Coordinate execution of steps to conversion with center to increase conversion ratio Establish and maintain sound working relationships with center leadership and liaisons Utilize PCC CRM database to enter all referrals and outcomes Constant communication throughout the day assigned centers and liaisons to discuss clinical review of referrals, questions, comments and concerns around potential admissions and to satisfy those questions prior to patient admission. Support 24/7 admission coverage with team members within central office Embrace North Shore sales philosophy and abide by all policies and procedures Complies with laws and regulations applicable to position and act in accordance with Center’s Compliance Program Safetyo Knows and follows Center rules
o Follows Center dress and hygiene policies
o Demonstrates proper use of equipment. Reports equipment needs or repairs
o Follows Center smoking policies
o Reports and documents any incidents or accidents of residents, staff or visitors to the appropriate Center personnel
o Reports all hazardous conditions/equipment to Supervisor
o Performs duties, which may include transportation of residents, as assigned in Center Disaster Plan
o Uses required protective equipment
o Follows infection control standards, policies and procedures
Resident Rightso Knows Resident Rights. Helps the residents/patients exercise and/or protect their rights
o Reports resident/patient complaints to management
o Maintains confidentiality of resident/patient information
HIPAAo Follows and adheres to Center’s policies and procedures implementing HIPAA requirements for the privacy and security of protected health information
o Uses and/or discloses only minimum amount of Protected Health Information necessary to complete assigned tasks
o Reports all suspected violation of company’s HIPAA policies or procedures to Center Privacy Designee
Required Education and Experience:Current Wisconsin LPN license in good standing. Experience in assisted living, long-term care, or a related setting preferred. Strong communication, organizational, and leadership skills. Ability to work collaboratively with the RN and other healthcare professionals.

care or health care experience preferred
