On our General Surgery 5D Unit, you will be caring for Lutheran’s Bariatric, Neurology, Spinal, and Orthopedic surgery patients. As a Licensed Practical Nurse (LPN) on this unit, you will provide care under the direction of an RN and assist with data collection, medication administration and treatments. In this role, you can make a lasting difference in the lives of patients, all while enhancing your skills through hands-on experience and numerous educational resources offered within the Cleveland Clinic system.  

A caregiver in this role works days, 36 hours per week. 2 8's (7:00am - 3:30pm) & 2 12's (7:00am - 7:30pm)

A caregiver who excels in this role will:

Provide care under the direction of an RN for assigned patients.  

Assist with data collection, medication administration and treatments.  

Perform accurate and complete admission, ongoing assessments and discharges within the defined time frames. 

Assess and document patients' pain levels utilizing appropriate protocols. 

Collaborate with the RN on the formulation, implementation, evaluation and revision of patients' care plans. 

Notify the RN of any pertinent physical, psychosocial, and emotional findings. 

Provide direct care to assigned patients and complete hourly rounding and the rounding log as assigned. 

Delegate and evaluate the care administered by non-licensed personnel. 

Administer prescribed medications and treatments in accordance with licensure and approved nursing protocols. 

Apply knowledge of usual dosage, indications and side effects of medications administered. 

Utilize the 5 rights of medication administration and ensure that all medications are secured at all times. 

Utilize appropriate reporting mechanism for any medication errors, near misses, etc.  

Under the direct supervision of the RN: assess the educational needs of the patient and significant others considering cultural issues and other special concerns, collaborate with the RN assigned to the patient on educational needs of the patient, provide basic teaching and educational material to the patient/family on diagnosis, tests and procedures, safe and proper use of equipment, reason for and safe use of medications, food/drug interactions, injury prevention, self-care and nutritional needs, review discharge instructions as previously presented by the Physician/RN, and document teaching. 

Minimum qualifications for the ideal future caregiver include:  

Graduate of an approved School of Practical Nursing 

Current licensure and registration as a Licensed Practical Nurse (LPN) in the State of Ohio 

One year of clinical experience within the past three years OR graduation within the past six months of hire

Basic Life Support (BLS) Certification through the American Heart Association (AHA) within one year of hire 

Preferred qualifications for the ideal future caregiver include: 

One year of patient care experience 

For certain areas: Certified in medication administration and IV therapy as approved by the Ohio Board of Nursing

Physical Requirements:

Requires full body motion to move and lift patients, manual finger dexterity with good eye-hand coordination.

Involves extensive standing and walking.

Requires corrected vision and hearing to normal range.

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:
