The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.
The LPN/LVN assesses patient health problems and needs, takes vital signs, administers medication, develops and implements nursing care plans, and maintains medical records. May advise patients on health maintenance and disease prevention or provide case management. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Required Qualifications
Current licensure as an LPN or LVN but this position will be filling the Lead Medical Assistant role for the clinicCertified or Registered Medical Assistant – OR – current licensure as an LPN.Phlebotomy Experience.Medication/vaccine administration experience.3+ years of MA experience – OR – LPN experience.High school diploma or equivalent.CPR certified.This role is considered patient facing and is part of the company's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.This role is part of Humana’s Driver safety program and therefore requires an individual to have a valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.Preferred Qualifications
Current licensure as an LPN or LVN in the State of TN.Strong clinical background.Strong sense of engagement as a team player.Experience in a fast pace/high volume environment.Proactive, self-starter, and resourceful.1+ years of MA – OR – LPN experience in Primary Care Organization center.Value Based Care experience includes knowledge of HEDIS, medication adherence, EDAPS and CAHPS/HOS Patient Experience.Bilingual proficiency in English and Spanish - must pass proficiency exam prior to foreign language communication.Additional Information:
