 As an LPN/LVN, who will assess patient health problems and needs, takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records. May advise patients on health maintenance and disease prevention or provide case management.

Job Functions:

You will be responsible for leading a team of 4-5 medical assistants, providing guidance, support, and oversight to ensure the delivery of high-quality patient care and efficient clinical operations.

Works closely with the clinic care coach to provide clinical education and guidance to patients

Works closely with primary care doctors to serve patients by providing disease screening, administering immunizations

Records patient statistics

Participating in educational opportunities

Obtains health history and/or immunization status by interviewing patient and/or relative

Requirements:

Active LPN license

IV Certified

1-2 years’ experience as nurse in inpatient or outpatient clinic setting

Multi-tasking and organization skills

Preferred:

Bilingual (English, Spanish)

Phlebotomy experience

Experience with Electronic Medical Records, specifically eCW (eClinical Works)

Experience with HEDIS

Knowledge of primary care medicine requirements (state specific)

This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program.  If selected for this role, you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$44,900 - $60,200 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
