**Description** **LPN – Home Health** **Location:** Vancouver, WA **Schedule:** Full Time | Day Shift | 1.0 FTE **Hourly Range:** $28.36 – $42.03 (DOE, union-represented) Are you a Licensed Practical Nurse seeking a rewarding opportunity to make a difference in people’s lives from the comfort of their homes? PeaceHealth is seeking an LPN to join our compassionate Home Health team in Vancouver, Washington. As a key member of our interdisciplinary care team, you'll provide direct nursing care to home-based patients under the supervision of an RN. Your expertise and attention to detail will help patients manage their health conditions, recover from illness, and maintain independence. **What You’ll Do:** + Deliver patient-specific nursing care, including administering medications and treatments + Support care planning in collaboration with the RN and providers + Provide wound care, monitor vital signs, and assist with patient education + Maintain accurate documentation in accordance with department standards + Participate in triage calls and provide compassionate support to patients and families + Ensure timely communication and collaboration with care teams **What You Bring:** + Graduation from an accredited LPN program + Current LPN license in Washington + Basic Life Support (BLS) certification + Valid driver's license and auto insurance + Strong clinical skills, IV proficiency, and the ability to work independently + Excellent interpersonal, communication, and organizational abilities + Preferred: 1+ year of LPN clinical experience **Why PeaceHealth?** We’re a mission-driven healthcare organization committed to providing high-quality, patient-centered care.
