Position Overview

Join our Home Health team as a Licensed Practical Nurse (LPN), delivering compassionate, high-quality care to patients in the comfort of their homes. Working under the supervision of an RN, you’ll play a key role in supporting patient outcomes, coordinating care, and empowering patients and families through education.

Key ResponsibilitiesDeliver skilled nursing care in accordance with individualized care plans under RN supervisionMonitor patient conditions, recognize changes, and communicate updates to the care teamCoordinate care with interdisciplinary team members to ensure continuity and optimal outcomesProvide patient and family education on disease management, treatments, medications, and safetyAccurately document care to support clinical, regulatory, and reimbursement requirementsParticipate in case conferences, team meetings, and ongoing care planningPromote patient independence and engagement through education and supportMaintain compliance with organizational policies and quality/safety standardsRequired QualificationsActive LPN license in the State of Illinois (or eligible)CPR CertificationPharmacology certificationMinimum 2 years of Med/Surg clinical experienceValid driver’s license and reliable transportationPreferred ExperiencePrior home health experienceFamiliarity with Medicare/Medicaid home health guidelinesSkills & StrengthsStrong clinical assessment and critical thinking skillsExcellent communication and documentation abilitiesAbility to work independently and manage time effectivelyComfortable using EMR systems and basic Microsoft Office toolsStrong interpersonal skills with a patient-centered approachWork Environment & RequirementsTravel throughout assigned geographic area to patient homesAbility to lift up to 35 lbs (with assistance/equipment as needed)Frequent standing, walking, bending, and patient assistanceWork performed in patient homes and community settingsCompensation

💵 $23.60 – $35.30 per hour (based on experience)
