LPN - Henrietta Medical Campus - Per Diem

SUMMARY:
The Licensed Practical Nurse is responsible for providing direct patient/resident care under the direction of a Registered Nurse, in accordance with established plans. The nurse must provide superior patient/resident/family customer service to align with the overall expectation of a positive patient/resident experience.

RESPONSIBILITIES:

Contributes to the identification of basic physical, emotional, and cultural needs of the patient/resident through observations and interactions.Observes and documents signs and symptoms related to the patient’s/resident’s health status and any changes in condition & Reports to the registered nurse.Provides routine nursing services, information, and education to patients/residents during routine care.Complies with evaluation, treatment, and documentation guidelinesObserves signs and symptoms, treats pain or discomfort, and documents response to treatment provided.Collaborates with appropriate registered nurse to revise care pathways when the patient’s/resident’s needs are not being met or have changed.Provides regular patient/resident status updates to the appropriate personnel.Attends and participates in department/facility meetings as required.Acts in compliance with hospital, regulatory, and professional standards and policies.Maintains clinical competency by pursuing continuing education opportunities.
 

REQUIRED QUALIFICATIONS:

Graduate from an accredited School of Practical Nursing.Current licensure in the State of New York.Dialysis Only - BLS (Basic Life Support) Certification required
 

PREFERRED QUALIFICATIONS:

1 year of experience preferred.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
