LPN - Cardiology Specialist

The LPN – Cardiology Specialist is dedicated to the health and well-being of our patients. The overall duties and responsibilities of this role may vary slightly based on specialization and from setting-to-setting, depending on the market and size, location and patient volume of his/her assigned center and/or Specialist. They will guide patients through complex and routine medical and social challenges in a manner that fosters trust and respect between the patient and the care team. The LPN Specialist is an integral member of a Physician-led care team that is focused on providing excellent and comprehensive primary care for a specific population of patients.

LPN Cardiology Specialist will play a crucial role in supporting our mission to urgently pursue more good days for our patients by preventing hospital visits through proactive cardiac care management.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

Patient Care & Monitoring:

Assist with cardiac-focused patient visits and assessmentsMonitor vital signs, especially blood pressure and heart rate patternsSupport EKG administration and basic interpretation under RN/provider supervisionHelp with medication reconciliation for cardiac medications

Core Cardiac-Specific Skills:

EKG Competency:

12-lead EKG placement and acquisitionBasic rhythm recognition (normal sinus, atrial fibrillation, bradycardia, tachycardia)Understanding of common cardiac abnormalities in our senior population

Vital Signs & Assessment:

Accurate blood pressure measurement techniques (manual and automated)Recognition of orthostatic changesPulse assessment including irregularitiesOxygen saturation monitoringWeight monitoring for fluid management

Point-of-Care Testing:

INR testing and documentation for anticoagulated patientsBasic metabolic panelsBNP/NT-proBNP testing when availableGlucose monitoring

Patient Relationship Building:

Help build trust between patients and the care teamProvide respectful, dignified care to our senior populationSupport patients with limited resources in understanding their cardiac care planUse 6th-grade level communication when educating patients

KNOWLEDGE, SKILLS AND ABILITIES:

Exceptional knowledge and understanding of general Practical Nursing protocols, functions, practices, processes, policies & proceduresKnowledge and understanding of medical products, terminology, services, standards, policies and proceduresExceptional knowledge and understanding of safety and sanitation protocols. Must be aware of personal and patient safety at all timesAbility to effectively collaborate with team members, including physicians, clinicians, market leaders, center managers and front desk team staffAbility to manage complex patients requests from providersAbility to make some independent workload decisionsAbility to maintain effective and organized systems to ensure timely patient flowAbility to exercise patience, compassion and empathy for patients and family membersAbility to act calmly in busy or stressful situationsSkill in operating phones, personal computer, software and other IT systems. Must be detail-oriented to ensure accuracy of reports and dataExcellent clinical skills to identify, diagnose and appropriately resolve patient issuesExcellent oral and written communication skillsGood non-clinical skills to assist with dressing, physical exercise, restroom visits, etc. as neededGood time management to ensure tasks are completed timely and efficientlyGood motor skills to effectively calibrate equipment, manipulate intravenous equipment, administer injections, position and transport patients when neededMindset focused on solving problems for patients and achieving team goalsProficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation softwareAbility and willingness to travel locally and regionally up to 50% of the timeSpoken and written fluency in English

EDUCATION AND EXPERIENCE CRITERIA:

High school diploma or equivalent education (GED) requiredActive LPN license in the state of practice1 year of LPN experience REQUIRED1 year of Cardiology experience preferred3 years of clinical experience preferredMedication reconciliation certification preferredCardiac rehabilitation support training PreferredBasic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
