Health Advocates Network is currently seeking an **Licensed Practical Nurse** to work at a facility in **New York, NY.** These are _registry_ positions with our company. **$100 Sign-On Bonus** **Pay Rate:** $40 / hour *W2 **Shift details:** + Night shift 11:00pm-7:30am + Day shift 7:00am-3:30pm + Evening shift 3:00pm-11:30pm LPN Duties to Be Performed A. Provide skilled nursing care to patients requiring professional nursing services. B. Implements client care by following Plan of Care or Doctor’s orders. Doctor’s orders may consist of: 1. Dressing changes 2. Insertion and irrigation of urinary catheters 3. Oxygen administration by mask or cannula. 4. Tracheostomy care and endotracheal suctioning 5. Gastrostomy tube feedings 6. Gastrostomy/Jejunum tube medication and flushes. 7. SQ or IM injections 8. Insulin administration 9. Obtain Specimens 10. Nephrostomy tube flushes 11. Colostomy care and/or irrigation 12. Administers CPR and other emergency procedures as necessary. 13. Provide staff training and in-services C. There is a need for short term/short time nursing that would come under private duty nursing or visiting nurses. These nurses would perform specialized services such as Diabetes Care (injection of insulin), wound care (complex wounds/wound VAC/pressure ulcers), Tube feedings, Trach care and many short term services that take a few hours or less. D. Participates in the development and review of habilitation plans/goal plans. Implements and provides documentation for plans. 1. Provides opportunities for Community inclusion utilizing community resources to develop personal relationships with members of the community. 2. Provides opportunities to make appropriate choices; offers suggestions for new choices. 3. Provides training and assistance to develop skills to promote self-reliance. 4. Provides opportunities for individuals to contribute to his/her own life and the lives of those around them, both in their community and their home. 5. Ensures timely documentation of activities in individual’s habilitation/medical notes. E. Consistently adheres to specific duties which are necessary to assure smooth program operations. 1. Participates in proper meal preparation & portion control according to prescribed diets & individuals’ preferences. Ensure proper food storage following proper sanitation principles. 2. Maintains a clean and homelike environment. 3. Follows Personnel and Medical policies and procedures; completes necessary documentation (ex: Time and Attendance records, shift reports, minor event logs, etc.) 4. Works cooperatively with and communicates appropriately with other team members, promoting a harmonious environment for the consumer. F.
