Key Responsibilities:
· Administer medications to clients via oral, injection, or self-administration methods based on service line protocols
· Conduct physical and mental health assessments, including psychiatric screeners
· Provide one-on-one support and build trusted relationships with clients
· Collaborate with providers by staging refill requests and addressing client medication needs
· Room clients, verify medical histories, and complete preliminary physical tasks (vitals, samples)
· Maintain complete and accurate documentation of all nursing care and interventions
· Oversee medication counts, monitor refills, and prepare pass medications for housing clients
· Act as a resource for medical questions, infectious disease control, and facility health issues
· Assist with general lab and phlebotomy tasks as needed
· Participate in meetings and contribute to team efforts across the care continuum
Education, Experience, and/or Credential Qualifications:
· Graduation from an accredited Practical Nurse program
· Current licensure as a Licensed Practical Nurse (LPN) in the state of Missouri
Additional Qualifications:
· Must have a current driver’s license, acceptable driving record, and current auto insurance
· Must successfully pass background checks including criminal, driving record, abuse/neglect, and fingerprint screening
· Strong communication and documentation skills (written and electronic)
· Ability to maintain confidentiality and handle sensitive information responsibly
· Basic math and problem-solving skills
· Willingness to collaborate, learn, and adapt in a team environment
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (up to 1/3 of the time) and/or a negligible amount of force frequently (1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may include walking or standing for brief periods. Jobs are considered sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
