Provide nursing care for a vulnerable adult population. Performs nursing assessment, treatment, education and health care service coordination to individuals and families. The LPN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services. Services will be provided at Community Healthlink and outreach locations.

I. Major Responsibilities:

1.  Assesses the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, medication administration and documentation.
2.  Provides appropriate health care and coordinates referrals as prescribed by providers.
3.  Maintains documentation required for all primary care clients receiving in-home nursing services.
4.  Promotes education and adherence to prescribed treatment protocols.
5.  Communicates with health care facilities when clients are admitted and plans for discharge to ensure continuity of care.
6.  Facilitates wellness education to individuals and offer groups with health and wellness focus.
7.  Records all client information within electronic medical record system daily, received by outside healthcare agencies.
8.  Administers immunizations per provider orders
9.  Performs daily quality control checks for the practice.
10. Assists RN Case Managers and medical assistants as needed on daily basis.
11. Active team member working on quality improvement within the Primary Care Practice sites.
12. Assists with population health screenings and outcomes.
13. Demonstrates person centered strength-based communications and relationship with consumers using recovery-oriented principles.
14. Maintains a knowledge base in accordance with the current trends of nursing and compliance with all standards of agency policies and procedures.
15. Demonstrates ability to communicate effectively and professionally with consumers, staff, and providers.
16. Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
17. Knowledge of medical, substance abuse and mental health nursing
18. Performs other related duties.

II. Position Qualifications:

License/Certification/Education:


Required:
1. Grade N02: Community LPN: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.

2. Driving is not a requirement.

Experience/Skills:
Required:
1. Must be able to pass a CORI background check.

Preferred:

1. 2-4 years of experience in medical nursing.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

1. Must be able to be in a standing position for extended periods of time.
2. Must be able to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medicals codes or alarms).
3. Must be able to lift and move objects up to 50 pounds for various needs.
