Licensed Social Worker (LSW) – Behavioral Health / Medical Social Work Location: 1903 Marquette Ave Suite A111 Muskegon, MI 49442 Hours per Week: 40 hours About the Role The Licensed Social Worker collaborates with an interdisciplinary team to support patients and families through assessment, care planning, advocacy, and resource coordination. This role focuses on promoting emotional, social, and overall well-being by identifying needs, connecting individuals to appropriate services, and supporting them through complex medical, behavioral, or social challenges. The LSW plays a key role in discharge planning, crisis intervention, and ongoing patient support.
