Licensed Social Worker (LSW) – Behavioral Health / Medical Social Work Location: 1903 Marquette Ave Suite A111 Muskegon, MI 49442 Hours per Week: 40 hours About the Role The Licensed Social Worker collaborates with an interdisciplinary team to support patients and families through assessment, care planning, advocacy, and resource coordination. This role focuses on promoting emotional, social, and overall well-being by identifying needs, connecting individuals to appropriate services, and supporting them through complex medical, behavioral, or social challenges. The LSW plays a key role in discharge planning, crisis intervention, and ongoing patient support. Key Responsibilities + Conduct psychosocial assessments and identify patient needs + Develop individualized care plans and coordinate services + Provide counseling, support, and crisis intervention as needed + Collaborate with interdisciplinary teams to support patient goals + Advocate for patients and connect them with community resources + Maintain accurate documentation and adhere to confidentiality standards + Support the workflow and goals of the social services department Minimum Requirements + Bachelor’s or Master’s degree in Social Work from an accredited institution + Active Licensed Social Worker (LSW) or Registered Social Worker (RSW) per state requirements + Minimum 1 year of social work experience preferred + Current CPR certification if required + Must meet all federal, state, and local employment requirements + Additional requirements: TB screening, health certificate, onboarding compliance Compensation Packages Local Licensed Social Worker Pay Rate: $45/hr Estimated Weekly Pay: $1,800 Travel Licensed Social Worker Hourly Pay: $22/hr Meals: $450 Housing: $750 Estimated Weekly Pay: $2,080 Facility Information Muskegon County is a healthcare and social services organization offering: + Behavioral health support + Medical social work services + Discharge planning and care coordination + Community resource linkage + Patient-centered, interdisciplinary care Certifications/Accreditations: State-licensed social services provider Notable Features: + Collaborative team environment + Diverse patient population + Strong focus on advocacy and support Why Join This Assignment This role offers the opportunity to support individuals and families during critical moments, providing meaningful interventions that improve quality of life.
