The LPN GS-7 will demonstrate the advanced knowledge and competence required to independently prioritize, organize, and carry out assigned duties with minimal directions and/or assistance of a registered nurse, except for reporting and seeking advice on any unusual situations which do not have clear precedents Responsibilities Patient Care: The LPN serves as the care coordinator, monitoring weekly appointments and staffing, LPN alerts Nurse Manager (or designee) when conflicts arise. The LPN successfully and consistently demonstrates exercise of independent technical judgment as well as comprehensive and thorough working knowledge of the most complex or difficult practices and procedures. The LPN is assigned the duty of patient care by conducting specific observations and examination procedures, generating clinical documentation, and performing diagnostic and therapeutic tasks according to the criteria established by the supervisor and local Medical Center policy. The LPN demonstrates nursing practice with supervision of a registered nurse or physician. Demonstrates the ability to assist the RN and/or physician to appropriately prioritize overall patient care needs and adjust plans for care delivery. Makes astute observations of subtle, less obvious changes, physical, mental, and emotional, in patient's condition, and accurately and thoroughly documents care delivered and patient's response to care that could require higher-level intervention. At this grade level, the LPN Telehealth Clinical Technician (TCT) is expected to participate to a greater degree in prioritizing, and as appropriate, modifying patient care treatment. The LPN administers medication accurately under the supervision of a registered nurse. Demonstrates understanding of the effects and/or reactions caused by medications, treatments, and procedures. Records and reports these observations. The LPN TCT demonstrates the independent technical knowledge and skills that are complete in nature and that contribute to the Veteran's treatment plan and support the successful completion of the Telehealth visit. The LPN TCT performs Telehealth imaging, including tele dermatology, and other Telehealth programming as technology becomes available, as identified and referred by the primary care provider, who retains responsibility for patient care. Appropriately seeks assistance and guidance by informing RN or MD/DO of changes in patient's condition requiring higher-level intervention. At this grade level, the LPN/LVN is expected to participate to a greater degree in prioritizing, and, as appropriate, modifying patient care treatment. They must also possess the ability to observe more subtle physical and emotional changes in patients and demonstrate taking appropriate action based on those observations. Ability to proactively assist the RN or MD/DO in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments according to established policies/procedures and appropriately recording administration and patient's response to medications/treatments received. (This may include restricted medication locally authorized for administration by employees at this grade level.) Demonstrates the knowledge and ability to readily recognize the more subtle signs and symptoms of potential or actual drug reactions, promptly inform the RN or MD/DO of observed changes in patient's condition and initiate appropriate, prescribed interventions. Employees at this grade level demonstrate broader knowledge and skill with respect to these responsibilities than employees at lower grade levels.
