Shift: Monday – Friday, 8:00 A.M. – 5:00 P.M.

Key Responsibilities:
Administer medications via oral, injection, and other approved methods.
Conduct physical and mental health assessments and psychiatric screenings.
Document medication administration and nursing assessments accurately.
Perform MAR reviews, medication counts, and manage house stock and refills.
Communicate with clients regarding medication needs and coordinate refill requests.
Prepare clients for appointments by verifying medical history and collecting vitals.
Maintain timely documentation of nursing interventions and client safety plans.
Support lab and phlebotomy duties as needed.
Serve as a resource for infection control, hygiene, and general health concerns.
Participate in team meetings and contribute to continuous improvement efforts.
Education and/or Experience Qualifications:
Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
Graduation from an accredited practical nursing program.
Additional Qualifications:
Strong verbal and written communication skills.
Proficient in EHR systems and documentation practices.
Ability to interpret lab results, clinical notes, and procedural manuals.
Skilled in basic math and medication dosage calculations.
Demonstrates sound judgment, adaptability, and a collaborative mindset.
Committed to confidentiality and secure handling of sensitive information.
Comfortable working in a fast-paced, client-centered environment.
