Martin North Hospital's 4NW Medical Renal unit provides care to med/surg patients with kidney disease, high blood pressure and other kidney-related issues. As a Licensed Practical Nurse (LPN), you will be responsible for providing direct and indirect patient care to assigned patients as a member of the Patient Care Team. LPNs function with minimal supervision in the care of patients whose conditions are of varying degree of acuity. Under the direction of a Registered Nurse (RN), you will be responsible for gathering information necessary to complete the nursing process, medication administration, documentation of treatments and care, management of equipment, patient safety and monitoring of vital signs. **A caregiver in this role works nights from 7:00 p.m. - 7:30 a.m.** A caregiver who excels in this role will: + Provide direct and indirect care of assigned patients as a member of the "Patient Care Team". + Involve patient/family in the development of care plan. + Update care plan daily. + Prioritize and perform direct care to patients according to policy. + Explain and document procedures and interventions. + Identify and perform appropriately in emergent situations. + Assist multidisciplinary team with all processes related to patient care. + Delegate responsibilities to staff based on patient needs and staff competencies. + Utilize and document patient safety restraints per hospital policy. + Perform all IV skills including starts according to unit policy. + Accurately document medications on medication administration record (MAR). + Monitor patient's response to medications and report appropriately. + Report medication errors and adverse reactions per hospital policy. + Demonstrate knowledge concerning medications, including side effects and administer medications. + Monitor blood and blood products per hospital policy. + Verify patient ID per hospital policy. + Perform all PYXIS monitoring and counts per hospital policy. + Determine patient/family educational readiness. + Develop plan based on educational assessment and educate patients. + Document educational plan, intervention and outcome. + Provide patients with discharge instructions/education and facilitates a safe discharge. Minimum qualifications for the ideal future caregiver include: + Graduate of an accredited practical or vocational nursing program + Basic Life Support (BLS) through American Heart Association (AHA) + Florida Licensed Practical Nurse (LPN) license + IV Therapy certificate required within three months of hire + Knowledge of infection control practices + Knowledge of quality improvement practices Preferred qualifications for the ideal future caregiver include: + One year of experience in an acute care setting Our caregivers continue to create the best outcomes for our patients across each of our facilities.
