Job Description:

We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our care team. The LPN will work closely with residents, families, and interdisciplinary staff to ensure the health and well-being of our residents. You’ll play a key role in monitoring resident health, administering medication, and supporting daily living activities in accordance with state regulations and facility policies.

Responsibilities:

Deliver direct nursing care to residentsAdminister medications and treatments as prescribedMonitor and document resident conditions and report changes to appropriate personnelProvide leadership to caregivers and support staffCommunicate effectively with residents, families, and medical professionalsEnsure compliance with resident care plans and facility protocolsPromote a safe, clean, and comfortable living environment
