timesEDUCATIONCERTIFICATIONGraduate of an accredited LPN ProgramCurrent licensure in the state of ConnecticutMaintains clinical competency required for area of assignmentincluding Basic Cardiac Life SupportEXPERIENCEExperience with computer scheduling systems preferred ie Meditech and Ntierprise year experience working in a medical facilityCOMPETENCIESAble to communicate effectively in English ndash both verbally and in writingBasic computer skillsComfortable in a fast pacedconfidential medical environmentTeam oriented with excellent written and verbal communication skillsKnowledge of common safety hazards and precautions to establish and maintain a safe work environmentKnowledge of professional nursing theory regulations and practices giving and evaluatepatient careSufficient physical ability and mobility to work in an office setting to stand or sit for prolonged periods of time to occasionally stoop bend kneel crouch reach and twist to lift care push and or pull light to moderate amounts of weight to operate office equipment requiring repetitive hand movement and fine coordinationAbility to maintain working relationships and interact with patients and employees on a to reference lab for testing as directed by a physician or mid levelResponds to incoming patient treatment related phone calls Instructs patients and family regarding medications and treatment instructionsOverseesvaccine program following State guidelinesMaintainsreviews patient records charts and other pertinent informationMaintain workarea in a neat and organized mannerMaintain appropriate documentation of compliance with CLIA regulationsPrimary Participates in Patient Centered Medical Home Care Team collaborationsMaintain patient confidentiality and adhere to
