Sentara is hiring a FLEXI remote LPN Intake Coordinator to support Home Care Services.

Weekends and holidays 8am-5pm on an as needed basis

Overview

Evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients. Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues.

Education

Highschool Graduate or Equivalent

Certification/Licensure

LPN

Experience

Minimum 1 years of experience with clinical and customer service in a healthcare environment (required)Home Health, hospice, or intake coordination experience is highly preferred

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$17.31 - $28.85. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
