Responsibilities

Position also reports to the RN Supervisor as applicable. Administers nursing care to residents under the direction and supervision of the Unit Manager. Responsible for the delivery of resident care through the process of assessment, planning, implementation, and evaluation under the direction and supervision of the Unit Manager. Maintains established departmental policies and procedures, objectives, quality improvement programs, safety, environmental and infection control standards. Maintains confidentiality of all Nursing Home/staff information. Strives to incorporate the philosophy and mission of Our Lady of Peace (OLP) and to incorporate the Nursing Home's goal of: Quality care, in a personal manner with total regard for human dignity. Adheres to Facility Standards of Conduct and practices the Core Values of Service of the Poor, Reverence, Integrity, Wisdom, Creativity and Dedication. Represents OLP accurately and honestly. Acts with integrity and in the best interest of OLP. Identifies instances of non-compliance and takes appropriate action, including reporting violations of the CRP. Adheres to Facility policy or regulations. Business relationships are conducted in the best interest of OLP, free of gifts, favors or other improper inducements. 

 

Position Accountabilities and Performance Criteria.  The following are essential job accountabilities:


1) Administers and controls medication and treatments as ordered by physician. (25%).

Application of Knowledge Performance Criteria - Complies with standard precautions to reduce occupational exposure. Follows established protocols for treatments. Follows established protocols for narcotic and medication administration and resident observation.Planning/Timeliness Performance Criteria - Plans all work steps to ensure success; considers time, supplies and support required from other individuals and departments in setting up work plan and completing duties.Resident Relations Performance Criteria - Communicates pertinent information regarding resident's progress and needs to appropriate nursing home staff.

2) The Licensed Practical Nurse, monitors the medical condition of residents and reports changes to the Unit Manager. (20%).

Application of Knowledge Performance Criteria - Exhibits expertise and skill in understanding, meeting and satisfying the specific needs and behavior of the resident in all age groups. Communicates effectively with residents with or without language barriers or limitations. Demonstrates knowledge of complications associated with each resident. Demonstrates knowledge of proper resources when unsure. Identifies emergency situations and directs immediate care. Maintains and improves resident optimal level of functioning and nursing rehabilitation. Recognizes changes in patient's condition and asks appropriate questions to clarify the situation. Reports changes to Unit Manager.Management Performance Criteria - Organizes work to complete job tasks in a timely manner.Resident Relations Performance Criteria - Communicates pertinent information regarding resident's progress and needs to appropriate nursing home staff.

3) The Licensed Practical Nurse monitors and ensures accuracy of recording on resident medical records. Maintains all required reports, records, statistics as directed by the Unit Manager. (17%).

Application of Knowledge Performance Criteria - Follows facility protocol for admission documentation. Demonstrates skill in the nursing process. HIPAA Competency- Protects resident's privacy by insuring resident charts or items with resident data with protected health information (PHI) are not left unattended or left opened.Management Performance Criteria - Ensures compliance with quality improvement policies and procedures. Contributes daily to updating the nursing care plan. Documents significant assessments to update resident records and charts.Record Keeping Performance Criteria - Documents in a clear, complete and organized manner. Monitor staff and unit to ensure quality standards are maintained. Accurately documents resident/family teaching and resident/family response to teaching. Accurately completes resident discharge summary according to departmental guidelines. Completes logs and records promptly, recording all information that could be pertinent to others. Reports accurate results to achieve the best possible resident care. Prepare accurate and timely documentation during all shifts of duty as required by guideline protocols.

4) Under the supervision of the Unit Manager, the Licensed Professional Nurse provides direct resident care, evaluates outcomes and adjusts nursing care processes as indicated to ensure optimal resident care. (10%).

Application of Knowledge Performance Criteria - Performs job duties within legal and professional regulations and guidelines. Demonstrates ability to provide direct resident care, evaluates and adjusts nursing care processes as directed by the Unit Manager or Director of Nursing.Management Performance Criteria - Organizes work to complete job tasks in a timely manner.Collaboration Performance Criteria - Identifies when help is required in providing resident care and seeks is appropriately, recognizing own limitations.Resident Relations Performance Criteria - Maintains a safe, comfortable and therapeutic environment for residents and families in accordance with nursing home standards.

5) The LPN monitors and supervises the performance and follows the implementation of resident care by  the CNA's on his/her unit. (10%).

Management Performance Criteria - Insures CNA Care Plan is followed at all times. Insures there is Range of Motion and Ambulation progress. Monitors breaks and lunches, insuring employees on the unit are absent from the unit only at designated times and duration. Provides feedback of CNA work performance to upper management.

6) The Licensed Practical Nurse assists physicians with examinations, procedures and other processes related to direct resident care as directed. (9%).

Application of Knowledge Performance Criteria - Cooperates with other personnel within the Nursing Home to achieve department objectives and maintain good employee relations, interdepartmental objectives, and infection control policies. Maintains a safe, comfortable and therapeutic environment for residents and families in accordance with Facility standards.Collaboration Performance Criteria - Demonstrates attitude of cooperation and professionalism. Promotes positive customer relations and maintains a professional appearance as per facility dress code. Provides appropriate assistance to physicians during various medical procedures.Resident Relations Performance Criteria - Explains procedures to patients thoroughly, providesassistance when necessary, monitors patients' comfort and alleviates anxiety during procedures.

7) The Licensed Practical Nurse assists physicians, co-workers and families as needed in time of stress or crisis. (5%).

Application of Knowledge Performance Criteria - Acts as an advocate of the Patients Bill of Rights and assures resident/family understanding. Demonstrates Mission of Facility by providing quality care, in a personal manner with total regard for individual human dignity. Effectively identifies predictable problems and plans and provides resident care.Collaboration Performance Criteria - Maintains harmonious relationship with unit and co-workers, providing support and assistance to co-workers to ensure quality resident care.Resident Relations Performance Criteria - Communicates clearly and tactfully with residents and their families.

8 ) Enhances professional growth and development within the Nursing Home through participation in educational programs, current literature, inservice meetings, and workshops. (1%).

Application of Knowledge Performance Criteria - Attends continuing education courses related to area of specialization to remain current in the field. Performs successfully all skills assigned to this position's Competency Checklist. Understands and demonstrates knowledge of the New York State Nurse Practice Act, the codes of the New York State Department of Health.

9) Attends all inservices and meetings as required. (1%).

Application of Knowledge Performance Criteria - Competency: Participates in drills and demonstrates knowledge of Plan for Department. Projects poise, authority, and a positive image. Wears appropriate clothing and accessories, giving proper attention to personal hygiene. Wears Facility ID Badge while on duty. Infection Control Competency: Demonstrates knowledge of and adheres to all infection control
procedures including proper hand washing techniques, contact with blood spills and universal precautions. Attends all mandatory inservices. Attends department meetings as required and scheduled.

 

The following duties may be reassigned:

 

10) Orients, instructs, and trains assigned personnel. (1%).

Collaboration Performance Criteria - Assists in the orientation, instruction and training of assigned personnel. Serves as a resource to co-workers.

11) Ensures an adequate stock of supplies and proper functioning of equipment. (1%).

Application of Knowledge Performance Criteria - Assists in the containment of Nursing Home costs through the appropriate ordering and conserving of supplies and equipment.

12) Performs other related duties as assigned or requested.


Requirements

Licensure / Certification / Registration:

Must possess Basic Life Support Certification credentials, required.Current New York State LPN License, required.

Education:

Graduate from an accredited school of Practical Nursing, required.

 

Work Experience:

On the job training provided.One (1) year medical-surgical skilled nursing home experience, preferred.
