Licensed Practical Nurse (LPN) – Assertive Community Treatment (ACT) Program
Location: Glen Cove, NY (Nassau County)
Schedule: Full-Time | Monday–Friday, 9:00 AM–5:00 PM
Compensation: $35–$37 per hour

We are seeking a compassionate and motivated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) team. This is a field-based role where you’ll work as part of a supportive, multidisciplinary team providing mental health and wellness services directly to individuals in the community.
Key Responsibilities:
+ Deliver mobile, community-based nursing care as part of a team supporting individuals with serious mental illness.
+ Support recipients in building life skills, improving self-care, and managing health-related goals.
+ Collaborate with ACT team members and assist medical staff in coordinating health and psychiatric care.
+ Develop and implement wellness-focused interventions based on OMH’s Wellness Self-Management (WSM) model and other best practices.
+ Advocate for clients, helping them access community resources and maintain stability in the least restrictive environment.
Requirements:
+ Active NYS Licensed Practical Nurse (LPN) license
+ Valid NYS driver's license and access to a reliable vehicle (travel required throughout Nassau & Suffolk Counties)
+ Experience or interest in behavioral health nursing, community nursing, or wellness promotion
+ Minimum of one (1) year of experience in mental health, substance use treatment, or health and wellness education preferred
