**Overview** We’re looking for a **Licensed Practical Nurse Continuous Care** to join our team. This position will report directly to the Executive Director/Administrator or Director of Clinical Services, but work under the direction of the Registered Nurse to provide continuous care visits to patients and families in a facility/unit or home setting.
