Licensed Independent Social Worker - Psychiatric

Serves as a member of the multidisciplinary treatment team. Coordinates discharge planning and community referral for psychiatric patients. Assists in networking with agencies in the service areas. Assists with marketing and performance improvement activities. Has proficiency working with the applications found within Microsoft Office Suite, including Microsoft Word, Excel, PowerPoint and Access._ **_Certificates, Licenses, Registrations_** _Licenses_ _Licensed Independent Social Worker or a Licensed Professional Clinical Counselor_ _Current CPR required or must obtain within 30 days of hire and maintain_ **_Education and/or Experience_** _Education: Masters_ _Masters Degree in Social Work/Counseling_ _Previous experience in networking with community resources._ _Three to five years previous experience in psych/mental health_ _Three to five years previous experience with addictions_ _At least 1 year of working with groups (inpatient or outpatient)._ **_Expectations_** _Develops with the multidisciplinary team and the client treatment plans, which address the fundamental needs of the client and facilitate improvement in mental status. Serves as a therapist for clients including completing assessments, performing individual, couple, family, group therapy and psycho-education classes along with treatment planning._ _Documents (same day as service) psychotherapy services directly provided and progress of individuals, couples and families. Contacts insurance companies to obtain benefit information, give pre-cert information and concurrent updates as required and documents this information in the computer. Acts as client advocate in relation to doctors and staff outside agencies. Writes letters and correspondence as indicated. Confers with multidisciplinary team daily regarding client status, progress in treatment, new physician orders, new concerns, recommended discharge plans._ _Attends workshops/seminars to maintain license and improve therapy skills. Enforces program rules and regulations._ _Participates in Performance Improvement activities as requested by coordinator. Collects and enters outcome data into computer and assists with aftercare follow up mailings. Attends unit meetings, generally on monthly basis. Attends quarterly division meetings. Participates on one of the Professional Clinical Practice Committees Abides by rules of St. Rita's Medical Center. Abides by Professional Code of Ethics of Ohio Counselor and Social Work Board. Complies with departmental and Medical Center policies and procedures. Able to work a flexible schedule. Demonstrates behaviors consistent with stated core values of Catholic Healthcare Partners.
