**THIS POSITION IS NOT A HOSPITAL POSITION. IT IS A TELEWORK POSITION AND REQUIRES LIMITED TRAVEL THROUGHOUT THE REGION**
Coverage area for this position includes but is not limited to the following counties in Region 6: Muscogee, Harris, Coweta, Carroll, Talbot, Randolph, Butts, Fayette, Henry, Lamar, Meriwether, Spalding, Houston, Peach, Chattahoochee, Clay, Crawford, Crisp, Dooly, Heard, Macon, Marion, Pike, Quitman, Schley, Stewart, Sumter, Taylor, Troup, Upson, Webster
HOURS: Monday-Friday 8am-5pm; no weekends, no overtime, state holidays off
Please note: This position will require a computer skills test and the ability to demonstrate proficiency using computer equipment and software.
Job Description and Responsibilities:
-Complete initial and annual Level of Care (LOC) determinations for NOW and COMP waiver participants.
-Complete Department of Medical Assistance Form 6 (DMA-6) and Department of Medical Assistance Form 7 (DMA-7) reviews. Upon initial placement into services, review the DMA-6 utilizing the available documents. On an annual basis, review the DMA-7 utilizing the available documents. Ensure completeness and quality. Submit DMA-6 to Georgia Medical Care Foundation (GMCF) for approval.
-Assist nurse manager with follow up and resolution of critical incidents in the Developmental Disabilities Clinical Oversight (DDCO) application for clinical oversight as assigned.
-Complete medical reviews for verification of medical support levels as assigned.
-Provide customer service to various individuals that we serve, as well as family members, caregivers, and support people.
-Make recommendations to be carried out by interdisciplinary team regarding identified clinically necessary services and support for the individual. Communicate well (frequent, focused) with all interdisciplinary team members for the good of the individuals served.
-Analyze problems by evaluating available information and resources and develop effective, viable solutions to problems.
-Maintain and ensure confidentiality of individual protected health information (PHI).
-Respond appropriately to inquiries.
-Communicate and work cooperatively with agency personnel, service providers, support coordinators and other interested parties to coordinate effort, resolve operational problems, and improve quality of consumer care.
-Accurately identify, measure, evaluate, and resolve deficiencies found in the consumer care process in a timely manner.
-Maintain all required certifications and licensure, including but not limited to, Georgia professional nursing license, Georgia driver's license, and BLS/CPR certification.
-Participate in all required training and continued education activities.
-Attend mandatory staff meetings/trainings as indicated by manager.
-Thoroughly complete and submit to manager all mandatory reports in a timely manner as indicated by manager.
Perform the following tasks on as needed basis as assigned by nurse manager:
-Complete nursing assessments or updates as needed and in accordance with established standards and within specified timeframes. Assessments require conducting interviews with individuals, family members, legal guardians, caregivers, and providers as needed.
-Perform initial and participant-directed Health Risk Screening Tool (HRST) assessments of individual health care needs as needed based on data collected through history, observation, and analysis of diagnostic data.
-Additional duties as assigned.
