We want your knowledge and expertise! **Job Summary and Qualifications** POSITION SUMMARY: Under the direct supervision of the Case Management Director, collaborates with the Case Managers, Social Workers, hospital personnel, and physicians to ensure continuity of care and effective discharge planning with positive outcomes. Implements policies and procedures to accomplish the mission and objectives for social work. Participates in activities with community agencies, organizations, or groups to promote knowledge of the availability of social work services and opportunities for the health care consumer. Demonstrates the ability to be an advocate for and strive to protect the health, safety and rights of the patient RESPONSIBILITIES: 1. Ensures continuity of care and effective communication of activities with emphasis on coordination of resource management activities. Coordinates discharge planning meetings with nursing and ancillary staff on individual nursing units to promote ongoing review of patient care activities related to provision of services, patterns of care, and utilization of resources. 2. Collaborates with the Director of Case Management to implement policies and procedures to accomplish the mission and objectives for the Social Work function. Assists Social Workers to identify patient and family problem areas and develops appropriate treatment plan with Social Workers as needed. 3. Collaborates with the Director of Case Management to ensure adequate Social Work coverage for the hospital. 4. Assists the Director of Case Management in performing Annual Performance Reviews of Social Workers in a timely manner. 5. Develops and participates in the on-call program for Social Work staff. 6. Assists in organizing Social Work New Hire Orientation program. 7. Precepts graduate students in Social Work (if any). 8. Actively participates in appropriate medical staff and Hospital Committees where applicable. 9.
