Job Title: Lead Personal Care Assistant/Worker
Position Purpose:
The Lead Personal Care Assistant/Worker plays a vital role in supporting the personal care needs, health, and well-being of participants within the Trinity Health PACE program. This position ensures compassionate, high-quality care while assisting the department manager with staffing schedules and providing mentorship to team members.
Key Responsibilities:
Deliver personal care services including assistance with toileting, grooming, eating, and mobility to promote participant independence.Support nursing care and monitor participant health, reporting changes in condition to the care team.Assist with recreational activities, meal service, housekeeping, and transportation needs.Maintain accurate documentation of services provided and participant progress.Ensure compliance with safety, infection control, and ethical standards.Lead by example in demonstrating Trinity Health’s Mission, Vision, and Values.Provide orientation and guidance to new and existing staff, fostering a collaborative and respectful team environment.Participate in interdisciplinary care planning and attend required training and meetings.Assist with scheduling to ensure adequate staffing coverage.Qualifications:
High school diploma or equivalent required; CNA certification preferred.Minimum one year of experience with elderly or frail populations; training provided if less.Strong communication skills and ability to work independently.Proficiency in Microsoft Office and electronic documentation systems.Ability to adapt to change and maintain confidentiality.This role requires compassion, reliability, and a commitment to enhancing the lives of participants through dignified, person-centered care.
