Job Summary and Responsibilities

Are you a compassionate and skilled Social Worker looking to impact lives? Join our dynamic Care Coordination team and provide vital support to patients and families in our hospital and emergency department.

In this role, you will:

Provide crisis intervention, psychosocial assessments, and counseling.Advocate for patients and families, connecting them with essential resources.Facilitate complex discharges and transitions of care.Collaborate with an interdisciplinary team to ensure optimal patient outcomes.

Schedule: Monday - Friday 8am - 4:30pm

Job Requirements

We're looking for someone with:

Master's degree in Social Work (CSWE accredited)2+ years post-MSW experience (5+ years preferred)Oregon LCSW or CSWA working towards licensure (LCSW required within 2 years)Excellent communication, problem-solving, and teamwork skillsWhere You'll Work
