Responsibilities

Complete investigations including interviewing program staff and persons supported, collection and review of relevant documentation, and determination of investigative findings.Document investigative findings consistent with in OPWDD regulations using Form OPWDD 149 consistent with best practicesManage case-specific investigations’ process and documentation in OPWDD’s Incident Reporting Management Application (IRMA) as well as in any relevant, internal systems or tools.Work with the AVP of Investigations and Compliance to develop client specific recommendations as well as program and system level recommendations based on investigation findings and incident management trends.Meet with program staff to discuss identified findings and recommendations and develop corrective action plans as needed. Provide technical assistance and monitoring of corrective action plans as needed.Travel to all agency sites across NYC’s five boroughs and in Westchester, Rockland, and Orange counties as needed for interviews, site inspections, or meetings with program staff.Staff need to be flexible for hybrid work (as determined); whereby working partly in the physical workplace, and partly remote-at another workspace or at home (as applicable) utilizing technology and equipment provided by the agency, including Teams to conduct interviews, collaborate with staff, participate in meetings and manage work; laptop connected to the internet; smart phones and the likeParticipate in the Incident Review Committee for the Developmental Disability (DD) Services division as well as the DD Special Review CommitteeSupport other compliance program functions as needed, including internal audit, program monitoring and trainingRepresent NYF at internal and external meetings and work groups as appropriate.Assist the Systems and Quality staff and leadership with special projects as needed.Participate in trainings provided by OPWDD related to Incident Management as needed.Must becertified to conduct Abuse/Neglect investigations by OPWDD and/or the NYS Justice Center.
