Job Posting: Intake and Triage Coordinator Location: Hybrid Industry: Healthcare Administration / Patient Services Position Overview We are seeking a detail-oriented and proactive Intake and Triage Coordinator to join our healthcare support team. This role is crucial in ensuring the accurate and timely intake, triage, and processing of patient referrals and associated documentation. The ideal candidate will have a strong understanding of healthcare systems, insurance protocols, and patient confidentiality standards. Key Responsibilities + Review and validate completeness and legibility of incoming patient forms. + Communicate with stakeholders to collect missing or unclear information. + Accurately transfer program-specific referral details into our software platform. + Ensure all forms and referrals are processed promptly in alignment with program requirements. + Create or update patient profiles to maintain data accuracy. + Evaluate form requests and assign appropriate services/resources per guidelines. + Report pharmacovigilance concerns (e.g., Adverse Events) per client protocols. + Triage referrals and delegate tasks to case managers according to program workflows. + Collaborate closely with internal teams to manage and resolve process issues. + Maintain strict confidentiality of all patient and proprietary data. + Complete all corporate policy training by required deadlines. + Adhere to Standard Operating Procedures (SOPs) with consistency. + Support miscellaneous projects or tasks as assigned.
