Connecticutrsquos most comprehensive healthcare network as an Intake SpecialistThe OSO Intake Specialist is responsible for managing and processing referrals received through Internal Referral Portals IRP and External Referral Portals ERP as well as community referrals for Home Health and Hospice services This role ensures obtaining accurate intake documentation timely communication with referral sources and efficient data entry into the EMR system to support smooth patient onboarding and care coordination to the local region intake teamsbullMonitor and process incoming referrals via IRP and ERP Forcura Fax systems and community referral platformsbullReview referral documentation for completeness accuracy and compliancebullEnter referral data into the Provider Link and HCHB EMR system toolsbullEffectively communicate across multiple referral platforms and correspondence streams email faxing etcbullEffectively communicate with Business Development Navigators and TCCseTCCs teams and community referral sources bullEstablish effective processes to quickly obtain timely referrals obtain missing documentation or collaborate to obtain needed referral informationbullCoordinate with OSO clinical intake specialists to complete both financial and clinical clearance to ensure timely assignment and scheduling at the region levelbullVerify payer source and in network insurance acceptance to confirm st level financial clearance and referral process successbullMaintain accurate logs and tracking systems for referral status and follow upbullEnsure compliance with HIPAA and internal policies regarding patient informationbullEscalate urgent or complex referrals to appropriate clinical and operational leadership teams on both the OSO and region levelsbullCollaborate with internal departments Authorization Billing scheduling to support smooth transitions of careStrengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high quality service Administrative Healthcare Experience years of experience preferably in Home Care preferredKnowledge Skills and Ability Requirements bullFamiliarity with IRP and ERP and community referral systems and workflowsbullKnowledge of HIPAA and patient privacy regulationsbullStrong data entry and documentation skillsbullExcellent verbal and written communicationbullOrganizational and time management skillsbullAbility to work in a fast paced detail oriented environmentbullCollaborate effectively with clinical and non clinical teamsbullPrioritize tasks and manage multiple referrals simultaneouslyWe
