Medical and dental benefitsShort- and long-term disabilityLife insurancePaid time off401(k) programFlexible Spending Account (FSA)Employee Assistance Program (EAP)Vendor discounts Responsibilities Complete the intake process from initial referral contact to home care admissionProvide direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providersUtilize clinical knowledge to triage incoming referrals to determine necessity of timeline initiation of careCommunicate directly with patients and families to ensure acceptance of services and coordinate the insurance verification and authorization processesDocument pertinent information regarding the patient’s demographics, medical condition, medications and treatments, and service being requestedCommunicate home care criteria to referral sources and serve as a resource for home care criteria and alternatives to internal and external customersEnsure face-to-face visit has been arranged if neededReport problems of a clinical nature to the Director of Clinical Operations or Clinical Supervisor as appropriateEstablish and maintain positive working relationships with current and potential referral sourcesEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Registered Nurse or Licensed Practical Nurse with current licensure to practice in state of operation2 years healthcare experience, home health experience preferredWorking knowledge of insurance reimbursement guidelinesCommitment to clinical and documentation excellence About our Line of Business Adoration Home Health and Hospice, an affiliate of BrightSpring Health Services, provides quality and compassionate services in the comfort of home, providing support for patients, families, and caregivers in their time of need.
