participate in their own care and recoveryProgram SummaryNatchaug Hospital Clinical Day Treatment Schools are located at six sites throughout Eastern Connecticut Students are referred and placed by Local Educational Agencies LEAs to provide education to these students on a short or long term basis dependent on school needs Natchaug CDT offers special education services to students in grades ndash whose social emotional or behavioral health problems prevent them from functioning successfully in a regular school environment Each studentrsquos individual academic and clinical treatment plan is designed to address their special needs and return them to their regular schools as soon as possibleJob SummaryCollaborates with studentsrsquo home districts to provide progress updates and gather student assignments Supports students with work from their home schools andor prepares work to be completed while on the inpatient unit at Natchaug Hospital Collaboration and consultation with the manager and unit staff to uphold an optimal level of support materials and resources for each programIndependently plan prepare and deliver lesson plans and instructional materials that facilitate active learning based on variable student needsProvide assistance toward completion of Public School driven assignmentsMonitor progress of each student and provide verbal and written feedbackPrepare
