Inpatient Pharmacist

Job Summary and Responsibilities

As our Pharmacist, you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury.

Every day you will meticulously review every prescription in your queue for safety, critically assessing for potential drug interactions, correct dosing, patient allergies, or any irregularities. You'll then authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station, ensuring expert oversight in every process.

To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for real people. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.

Intervenes when appropriate with questionable medication orders and resolves problems with the prescribing physician. Documents results in the Pharmacy Intervention Program. Prepares compilations and summaries of data when assigned.Monitors patients in the various patient focused care Quality Improvement Programs such as the kinetic dosing program, monitoring total parenteral nutrition profiles, adverse drug reaction reporting program, the renal dosing program and other clinical programs as assigned. Provides timely response to resolve patient care issues. Compiles and presents data analysis when assigned.Provides drug information to the medical and nursing staffs including the status of non-formulary drugs, drug interactions, medication dosing, and other aspects of pharmaceutical care. Documents information exchange.Dispenses take-home prescriptions to inpatients being discharged and outpatients using the outpatient pharmacy computer system, when appropriate according to hospital policy. Provides counseling and written information as required to ensure safe use of medications.Reads, interprets, and enters physician medication orders into the computerized patient record with minimal errors. Accurately verifies electronically entered orders (CPOE) and assures that a clean and accurate eMAR is provided for our nurses to use.Ensures the accuracy and complete labeling of all dispensed medication and intravenous solutions leaving the pharmacy. Accomplishes this task by supervising the interns/technicians and their work areas. Assist interns in completing their board-required internship and acts as preceptors for tech student externs.
