Infusion Pharmacist Responsibilities:

Reviews and verifies all prescription medication orders received from the prescriber or provider’s agent and evaluates appropriateness and completeness of prescribed therapies in a home setting.

· Assesses the patient at the initiation of IV drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy.

· Ensures drug, related necessary supplies and all patient collateral (education materials) are accurately provided to our patients

· Facilitates communication with the patient/caregiver, physician, provider, home health nursing staff, and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure optimal clinical outcomes.

· When required, must be able to compound intravenous and specialty pharmaceuticals products using aseptic techniques according to established Florida Board of Pharmacy, federal agencies, USP
