Responsibilities
Comply with all state-mandated Occupational Therapy Practice Act, Occupational Therapy Code of Ethics and other legal mandates.
Evaluate client for therapy needs and provide written orders/treatment plan for services.
Ensure plan of care is consistent with client assessment and needs.
Instruct other personnel and/or family members and caregivers on intervention goals.
Ensure proper visit documentation completion.
Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider.
Participate in discharge planning..
Provide in-service education for agency personnel, as needed.
Ensure personal credentials are active and up to date at office.
Attend all staff meetings and complete all required annual education as assigned.
Requirements
Current unrestricted Occupational Therapist license in state for which care is being provided
One (1) year experience as occupational therapist, preferably in an in home setting
CPR Certification, required
Two-step negative TB skin test or chest x-ray, required.
Level 1 Fingerprint Clearance Card, required
Valid Driver’s License and auto insurance, required
Understands regulatory compliance with state and federal regulations
Adheres to HIPAA and maintains client confidentiality
Transportation to meet the needs of position and scope of job responsibilities, required
