There is currently one extra-help, part-time position within Behavioral Health & Recovery Services’ Alcohol & Other Drug Services (AOD) Unit. The position is embedded at the San Mateo Medical Center and will travel/drive clients throughout the County as needed. Will be required to work during peak hours, including nights and weekends, as well as some holidays. Primary responsibilities will include, but are not limited to, the following: + Engage individuals with severe and persistent substance use disorders to provide a range of services and supports, including but not limited to, substance use screening and brief intervention. + Assess clients' needs using assessment criteria to determine medical necessity and identification of key clinical interventions. + Engage clients using Motivational Interviewing and Harm Reduction practices. + Provide intensive case management, including transportation, client advocacy, client-driven treatment planning, and care coordination with other care providers. + Refer and connect clients to a continuum of substance use disorder and mental health services, including medication-assisted treatments. + Initiate a "warm hand-off" process to increase the likelihood of a successful engagement. + Provide education to clients and in the community on Medications in Addiction Treatment and harm reduction strategies. + Participate in a multidisciplinary team; use clinical consultation and case conferencing on difficult cases; and partner with key referral sources and other service providers including but not limited to, San Mateo Medical Center Emergency Department, Psychiatric Emergency Services, and Primary Care and Specialty Clinics, Behavioral Health and Recovery Services clinics and programs, Substance Use Disorder treatment providers, including Palm Avenue Detox, shelters, Probation, and Correctional Health. Ideal candidate will possess: + Significant experience and training in providing case management, assessment and evaluation of adult clients with substance use issues. + Experience working with adult clients having co-occurring mental health and substance use disorders, emergency services, criminal justice system, or adult clients, respective to the position. + Demonstrated experience in determining appropriate referrals and recommending treatment plans. + Ability to maintain case files in an electronic health record system. + Experience establishing appropriate clinical relationships. + Demonstrated understanding and experience with confidentiality and ethics. + Ability to work independently. + Excellent collaboration skills to work with multiple partner agencies. + Experience providing culturally appropriate client services. + Demonstrated understanding and experience with the issues of confidentiality and ethics. + Excellent communication skills, both written and verbal. + Experience using bilingual Spanish speaking skills on the job; fluency in Spanish is strongly preferred, but not required. + Certified Alcohol and Drug Counselors (e.g. CADC-I/II, CAADE) are strongly preferred, but not required. NOTE: This is an extra-help, at-will assignment, paid on an hourly basis. Extra-help hours are dependent on the business needs of the department and therefore work hours may vary from week to week. Extra help employees shall not exceed 1,040 hours of work per fiscal year. Some extra help positions are eligible for benefits under the Affordable Care Act. Extra help employees are not guaranteed permanent status at the end of the assignment. Qualifications Knowledge of: + Principles, philosophy, practices, materials, trends and literature related to substance use and mental health disorders, and impact of trauma. + Various models of treating addiction including medication assisted treatments, and the use of medications to treat substance use disorders, CBT approaches, relapse prevention and harm reduction. + How to work effectively with individuals with developmental disabilities and cognitive impairments, including those related to substance use. + Prevention methods and treatments procedures related to alcohol and drug disorders, mental and emotional illness, and personality disorders. + Social aspects of drug, alcohol, mental and emotional problems. + Techniques and dynamics of group and individual counseling. + Techniques and dynamics of assessment and engagement. + Standardized clinical assessment instruments, practices and procedures. + BHRS policies including informing clients of consent to services, notice of privacy practices, and OCFA right to file a grievance procedure. + Social and psychological needs, problems, behavior patterns and treatment of persons impacted by substance use. + Availability of and procedures for obtaining a wide variety of community and governmental services and resources. + Methods and techniques of interviewing, problem solving and motivational interviewing. + Principles of counseling, including group leader functions. + Resources/strategies for the client for purpose of referral. + Appropriate methods and use of assessment to evaluate and assess alcohol and drug disorders. + Basic case management processes. + Reporting requirements of criminal justice referrals and treatment providers. + Federal and State confidentiality and privacy regulations including 42 CFR part 2, HIPPA, etc. Skill/Ability to: + Assess client's immediate needs and ensure client's receipt of needed services. + Interview clients and other professionals to gather information. + Perform effectively in stressful, emotional, and confrontational situations. + Recognize factors causing reaction or changes in clients' condition or behavior. + Obtain, maintain, and present accurate client data. + Communicate clearly and effectively both orally and in writing. + Secure pertinent social and personal data and meet documentation requirements. + Develop relationships with the treatment provider community, courts, probation and other professional agencies/staff. + Evaluate information received and make appropriate treatment recommendations and/or referrals. + Establish and maintain effective working relationships with clients and their families, professionals and para-professionals and support staff in the department, outside agencies and the public. + Provide high-quality clinical case documentation. Education and Experience: Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying. A typical way to qualify is: Case Management/Assessment Specialist II: Two years of experience assessing and evaluating clients using assessment instruments and/or guidelines, identifying and making referrals based on those assessments, and interviewing clients for in a social service program.
