JOB SUMMARY

This job oversees the housing services and supports for members. The incumbent collaborates with federal, state and local affordable housing agencies such as US Department of Housing and Urban Development (HUD), the State Housing Authority subsidized housing, public housing authorities and voucher programs. The incumbent works with individuals and families in finding affordable, accessible housing through one of the above-mentioned agencies. Assists in educating and assisting the case managers regarding affordable, accessible housing services members.
ESSENTIAL RESPONSIBILITIES
Research, evaluate and analyze demographic and housing data in order to assist in identifying housing options for members.Identify member barriers and develop strategies for maneuvering through the barriers that exist to find suitable living situations.Act as a liaison for members, Case Managers and housing agencies in assisting individuals and families with finding affordable, accessible housing.Engage with members of the health plan face to face to support their housing needsUtilize the health plan’s system of record to document all interactions with and related to the health plan members.Serve as housing subject matter expert in affordable, accessible housing options.Develop standards, policies and procedures to define and support the overall program.Develop strong partnership and collaborate with community organizations and various federal, state and local government agencies such as (HUD) and the state housing authority in identifying and finding affordable housing options for members.Provide training and support to Case Management staff to assist in identifying members for transition into the community and to assist in the transition of members to safe, affordable, accessible, quality housing.Work with team members and management as needed to complete program deliverables.Evaluate and oversee housing initiatives and provide feedback and recommendations as needed.Other duties as assigned and requested.
