Hospital Pharmacist
Description The contracted pharmacist shall dispense drugs prescribed by physicians and other health practitioners and provide patients with information regarding medications and their use. The pharmacist shall advise physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications. The pharmacist must be able to evaluate provider medication orders for pharmacologic duplications, incompatibilities, side effects, drug–food and drug–drug interactions, and allergies. The pharmacist must be able to appropriately address potential therapeutic problems such as inappropriate dosage, incorrect strength, or improper dosing intervals. The pharmacist shall effectively communicate questionable therapies to providers and recommend reasonable alternatives in accordance with formulary guidelines established in the Computerized Patient Record System (CPRS). The pharmacist shall counsel patients and respond to questions regarding prescription drugs, including possible adverse reactions or interactions. The pharmacist shall also provide advice on durable medical equipment and home healthcare supplies. The pharmacist shall ensure that all medication orders are entered and verified in the electronic patient medical record (VistA). Pharmacist will be required to report specific patient outcome information, such as complications to the Service Line Chief. Quality improvement data provided by the pharmacists and/or collected by the Service will be used to analyze individual practice patterns. The Service Chief will utilize the data to formulate recommendations for the Professional Standards Board to consider in the renewal of clinical privileges.
