overall patient experienceThe Unit Lead works closely with nursing leadership case management and hospital medicine colleagues to support operational efficiency care coordination and quality patient concerns and care coordination challengesQuality Improvement Patient SafetyHelp drive unit based quality initiatives related toLength of stayReadmission reductionEarly discharge planningPatient experienceReduction of hospital acquired conditionsReinforce evidence based clinical practice and best care standardsParticipate in quality monitoring case reviews and improvement initiativesOperational ExcellenceSupport efficient utilization of hospital resources and care delivery processesIdentify and escalate barriers impacting patient care discharge planning or clinical workflowWork with hospital leadership to implement clinical and operational improvementsAdditional Leadership ResponsibilitiesParticipate in hospital medicine leadership meetings and unit triad
